Change Business Name and/or Address on Special Order Shipping License
Introduction:
If you are changing your business name and/or address on your Special Order Shipping license, complete the ABC-22 form.
If you are changing your business name and/or address on your Liquor Enforcement Tax Certificate, complete the DO-5 form.
If you are changing your business name and/or address on both your Special Order Shipping license and your Liquor Enforcement Tax Certificate, complete both forms.
Changing My Business Name and/or Address for My Special Order Shipping License:
The Marketing Unit within the ABC Division processes name and address change requests for licenses. Complete the ABC-22 form below and return it to the address at the bottom of the form.
Changing My Name and/or Address for My Liquor Enforcement Tax Certificate:
The Customer Relations Miscellaneous Tax Segment within the Division of Taxation processes name and address change requests for Liquor Enforcement Tax Certificates. Complete the DO-5 form below and return it to the address at the bottom of the form.
